Services and Costs
As part of my commitment to giving you the time you need and direct access to me, your provider, I do not go through insurance. I charge a low monthly membership fee which covers all needed doctor visits.
Membership Cost:
Pediatrics and Young Adult (Less than 25 years old) = $25/month
General Adult (25-64 years old) = $50/month
Senior Adult (65 years and older) = $75/month
Start up fee of $100 (will be charged on enrollment)

Direct Access to me, Dr. David Fields, by text, phone, or email
Unlimited in Office Appointments as needed Monday, Tuesday, Thursday, or Friday
Health Coaching and Counseling with Dr. Fields, trained in Lifestyle Medicine
Access to Dr. Fields’ Wellness Lectures both in person and online
