Services and Costs

As part of my commitment to giving you the time you need and direct access to me, your provider, I do not go through insurance. I charge a low monthly membership fee which covers all needed doctor visits.

Membership Cost:

• Pediatrics and Young Adult (Less than 25 years old) = $25/month

• General Adult (25-64 years old) = $50/month

• Senior Adult (65 years and older) = $75/month

• Start up fee of $100 (will be charged at first visit)

Services Included Free with Membership:

• Direct Access to me, Dr. David Fields, by text, phone, or email

• Unlimited in Office Appointments as needed Monday, Tuesday, Thursday, or Friday

• Health Coaching and Counseling with Dr. Fields, trained in Lifestyle Medicine

• Access to Dr. Fields’ Wellness Lectures both in person and online

Services available to Members at a low additional cost:

• Discounted Medications ordered wholesale by us

• Discounted Labs drawn at the clinic by us

• In Office Procedures at significantly lower costs than offered by hospitals, ER, or Urgent Care

• IV Hydration therapy in the clinic
